Order your Ataxia ID Card
Friends of Ataxia UK can request a free Ataxia UK ID card by completing the relevant form below.
- If you have ataxia, please complete the Living with Ataxia ID card form.
- If you are a family member, friend or carer of someone with ataxia, please complete the Family, Friends and Carers ID card form.
The Awareness and Symptoms ID card, usually for family, friends and carers, is available to any Friend of Ataxia UK. If you have ataxia but would prefer not to carry the Living with Ataxia ID card, you can request this card instead.
You will be asked to provide your full name and postal address so we can process your request and send your card to you.
Adding a photograph to your ID card
If you would like a photograph of yourself included on the front of your ID card, you can upload a digital photo when you complete the form. Alternatively, you can email your photo to volunteering@ataxia.org.uk, using your full name in the subject line. For example: “Ataxia UK ID card – Jane Smith photo”.
Your digital photo should be:
- clear and in focus
- in colour
- at least 600 pixels wide and 750 pixels tall
- at least 50KB and no more than 10MB
- taken against a plain, light-coloured background
- of you only, with no other people or objects in the photo
If you email your photo separately, please remember to include your full name in the subject line so we can match it to your request.
How long will it take to receive my ID card?
ID card requests are usually processed in batches, around twice a month. This means some cards may be sent out quite quickly, while others may take a little longer depending on when the request is received. Please allow up to 3 weeks for your ID card to arrive.
If we need any further information from you, such as a photo, this may delay the card being processed. Requests may also take a little longer during holiday periods or particularly busy times.
